A rule that cannot handle reality eventually gets bypassed

Governance requires rules because people need to know what is normally allowed, who has authority and which evidence is required. But no rulebook can predict every circumstance. The challenge is allowing legitimate exceptions without turning “exception” into a convenient way to avoid accountability.

Define the normal rule first

Preserve the evidence behind approval

Limit exceptions where appropriate

Some approvals should expire, apply only to one case or require review after a defined period. Otherwise a temporary exception can quietly become a permanent unwritten rule.

Review repeated exceptions

If the same exception appears repeatedly, the organization should ask whether the original rule is unrealistic. A mature governance system is willing to redesign a weak rule instead of approving the same workaround forever.
