One Decision Record: Why Authority, Evidence, Exceptions and Review Must Remain Connected

A decision without its context becomes difficult to govern
Approval records are often preserved as a signature, an email or a meeting minute. Months later, the institution may still know what was approved but no longer remember why, which evidence mattered or what exception caused the decision to change.

Begin with the question
A decision record should identify what needed to be decided. Without the question, later readers may see the answer without understanding the problem the organisation was trying to solve.
Evidence should remain attached to the decision
Financial information, technical reports, supplier evidence, risk assessments or professional advice may all contribute depending on the issue. The record should preserve which evidence was considered rather than simply storing documents in unrelated folders.
Authority should be explicit
Who could decide? What was the limit of that authority? Was escalation required? A governance system becomes more coherent when the approval is connected to the rule that made the approval legitimate.
Exceptions should live inside the same history
If the normal rule could not be followed, the exception should explain what condition changed, who approved the departure and whether the exception had limits or expiry conditions. This prevents temporary workarounds becoming invisible permanent rules.
Execution should be traceable to approval
A decision has limited governance value if nobody can later tell whether it was implemented as approved. The record should identify the owner and, where proportionate, the evidence expected at completion.
Review closes the reasoning loop
Outcome review asks whether the evidence was sufficient, whether assumptions proved accurate and whether the authority structure worked. The review should not erase the original decision; it should sit beside it as the institution’s later understanding.
