A policy can be carefully drafted and still have little effect on institutional behavior. GACM approaches governance operating integrity by asking what happens after approval: who owns the control, what evidence shows it is working, how oversight occurs and what happens when the process does not behave as intended.

Policy should define responsibility clearly

A useful policy establishes more than a principle. It should make authority, responsibility and escalation understandable enough for people to know what is expected when a real decision arrives.

Oversight needs evidence

Oversight cannot depend only on assurance from the people performing the activity. Reports, records, reviews or other evidence should allow the institution to test whether important controls are functioning in practice.

Accountability needs a visible owner

Controls fail quietly when everyone is responsible in theory and nobody is responsible in practice. A clear control owner helps the institution know who maintains the process and who should respond when the evidence shows weakness.

Review should challenge both performance and design

A review should ask whether the control was followed and whether the control itself remains appropriate. A perfectly followed process can still be ineffective if circumstances have changed or if the control protects the wrong risk.

Correction keeps governance alive

Governance operating integrity includes the ability to correct. When a control fails, the response should not be limited to repairing the immediate instance. The institution should ask whether authority, documentation, monitoring or the control design itself needs to change.

The difference between governance on paper and governance in practice is evidence. Policy sets the expectation. Ownership carries it. Oversight tests it. Review interprets the result. Correction strengthens what fails.

Governance controls can weaken when the people who designed them move on or when operational attention shifts elsewhere. Continuity requires the institution to preserve enough responsibility, evidence and review rhythm for the control to remain active beyond the individuals who introduced it.

That makes governance less dependent on personality. A control becomes institutional when the organization can still explain who owns it, how it is tested and what happens when the evidence shows that it is no longer working as intended.
