One Decision Record: Why Authority, Evidence, Exceptions and Review Must Remain Connected

A decision without its context becomes difficult to govern

Approval records are often preserved as a signature, an email or a meeting minute. Months later, the institution may still know what was approved but no longer remember why, which evidence mattered or what exception caused the decision to change.

Begin with the question

A decision record should identify what needed to be decided. Without the question, later readers may see the answer without understanding the problem the organisation was trying to solve.

Evidence should remain attached to the decision

Financial information, technical reports, supplier evidence, risk assessments or professional advice may all contribute depending on the issue. The record should preserve which evidence was considered rather than simply storing documents in unrelated folders.

Authority should be explicit
