Every important decision hides a governance question
Organizations often focus on what should be done while leaving another question implicit: who has the authority to decide? Governance becomes visible when a decision is challenged, delayed or produces an unexpected consequence.
GACM operates in governance, strategic advisory and management architecture. Its 14 August question is therefore direct: who decides, on what evidence and under which authority?
Who owns the decision?
Consultation can involve many people, but final ownership should be understandable. If everyone participates and nobody owns the decision, responsibility becomes difficult to trace afterward.
What authority does the decision owner actually have?
A title may imply seniority without defining limits. Which expenditure can be approved? Which exceptions require escalation? Which customer commitments need higher approval? Which decisions cannot be delegated?
Governance works better when those limits exist before urgency tests them.
What evidence is required?
Authority should not turn personal preference into institutional policy. Important decisions may require financial information, technical evidence, supplier records, risk assessments, professional advice or other inputs depending on the issue.
The relevant evidence should answer the decision question rather than simply increase the volume of paperwork.
Who must be consulted?

What requires escalation?
Escalation should be triggered by defined conditions where possible: value, risk, legal exposure, material scope change, conflict, security impact or another threshold relevant to the organization.
When escalation exists only as intuition, teams discover the real rule too late.
What happens when conditions change?
A decision may be correct when approved and become inappropriate later because evidence changes. Governance should allow reassessment without treating every change as failure.
Who reviews the outcome?
Review closes the accountability loop. Was the decision executed as approved? Did the evidence prove reliable? Were assumptions wrong? Did the authority structure work? What should change next time?
